MY AGED CARE

My Aged Care is a single point of entry into aged care services. It aims to give you more control and choice over your services.It is designed to help older people stay independent and in their homes and communities for longer.

We provide a Home Care Package (HCP) which is a more intensive, long-term package of aged care services to help people aged 65 years and over with higher care needs. These packages are ‘Consumer Directed’, meaning that you and the person you care for have a say in what is included in the package and how support is delivered.

To check your eligibility an assessment is carried out to evaluate your needs. You must be 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people.
